The short version: we collect what we need to coach you, we keep your health information confidential, and we never sell or share it for advertising.

1. What we collect

  • When you request a call: your name, email, optional phone number, age range, role, what you want to change, whether you have recent bloodwork, any wearable you use, and how you heard about us. With it, we receive the first page you visited on this site, the name of the site that sent you there, and any campaign tags in that link. Your browser holds those for the current visit only and sends them only if you submit the form.
  • During coaching: what you choose to tell us, which may include training history, food, sleep, stress, health conditions, medications, lab results or wearable data.
  • When you leave a review: your name, email, rating, what you are reviewing and what you wrote. If you agree, we publish the review with your first name and last initial. Your email is never published.
  • In the client portal: what you log there, such as meals, water, workouts, body measurements, sleep, mood, habits, goals and reflections, plus your messages with your coach, the time zone and units you choose, and a record of when you agreed to our terms. Your reflections stay private to you unless you choose to share them with your coach.
  • Automatically: our hosting provider keeps standard server logs, such as IP address and browser type, for security and reliability. This site sets no advertising or analytics cookies. Signing in to the client portal sets one cookie that keeps you signed in, and nothing else.

2. How we use it

To reply to you, schedule calls, keep track of our conversation with you in our sales records, deliver and improve your coaching, take payment, keep records we are required to keep, and keep the site secure. If you email us from a thread we started, the reply is filed with your record. That's all.

3. What we never do

We never sell your information, never share it for advertising, and never give it to the makers of any product we discuss. We do not use your health information for marketing.

4. Your health information

We are a coaching business, not a healthcare provider, so the federal health privacy law HIPAA generally does not apply to us. We still treat anything you share about your health as confidential: it is seen only by the coaches working with you, used only for your coaching, and shared with no one unless you ask us to or the law requires it.

5. Service providers

We use a small number of providers to run the business, such as website hosting, email delivery, video calls and payment processing. They handle information only to provide their service to us. Card details are handled by the payment processor and never stored by us.

6. How long we keep it

Call requests that don't become clients are deleted within 12 months. Client records are kept while you are a client and for as long afterward as we need them for legal, tax or accounting reasons, then deleted.

7. Your choices

You can ask to see, correct or delete the information we hold about you at any time by emailing baselineforum@gmail.com. We will respond within 30 days. Clients can also download everything they have logged, and request deletion, from Settings in the client portal. Deleting your information may mean we can no longer coach you.

8. Security

We use reasonable safeguards, including encrypted connections and access limited to the people who need it. No system is perfectly secure, and if a breach affects your information we will tell you as the law requires.

9. Adults only

Our services are for adults. We do not knowingly collect information from anyone under 18.

10. Changes

If we change this policy, we will update the date at the top. Our Terms of Service cover everything else. Baseline Health & Performance is operated by Leve AI Studios LLC, a Florida limited liability company.
